This course is intended for educators who work with students with disabilities ages 14-22. The course walks learners through all the components of the postsecondary transition plan, including the mandates, the compliance requirements and effective practice for creating high quality postsecondary transition plans, leading to robust postsecondary transition services which will improve the outcomes of the students with whom they work.

The primary learning intentions of this course include:
You will gain knowledge about the planning process for postsecondary transition assessments for students with disabilities.
You will learn about effective practices in choosing and implementing postsecondary transition assessments with your students.
You will learn how to align the rest of the postsecondary transition plan with the assessment results.
You will define, identify, choose, and apply your knowledge to your practice in your setting.
